our healthcare professionals, services, programs, membership offerings,
educational resources, and related business activities. The information provided through the Website is for general
informational and educational purposes only. It is not intended to
constitute medical advice, diagnosis, treatment, or a substitute for an
examination, consultation, or individualized care from a qualified
healthcare professional. Information on the Website should not be relied upon as a substitute

patient-provider relationship is appropriate and the applicable patient

department. Do not use the Website, email, text messaging, appointment-request

limitation: The availability and appropriateness of any service, treatment,
medication, procedure, program, or therapy is determined by the treating
healthcare professional based upon the patient's individual
circumstances and applicable law. Descriptions of services on the Website do not guarantee that any
particular treatment, medication, procedure, or service will be
recommended, prescribed, provided, or appropriate for any particular
person. SOZARA may provide healthcare services through telehealth when
clinically appropriate and permitted by applicable law. Florida law recognizes telehealth as including the use of
telecommunications technology to provide healthcare services such as
assessment, diagnosis, consultation, treatment, monitoring, and related
services. Telehealth providers are required to practice within their
applicable scope of practice and consistent with the professional
standard applicable to in-person care. Telehealth services may be subject to additional consent, identity
verification, technology, geographic, licensure, and clinical
requirements. The location of the patient at the time telehealth services are
provided may affect whether and how services can legally be provided.
SOZARA may decline or discontinue telehealth services when required by
law, professional standards, clinical circumstances, licensing
requirements, or other applicable restrictions. Submitting an appointment request through the Website does not
guarantee an appointment. An appointment is not considered confirmed until SOZARA or its
authorized scheduling system confirms the appointment. SOZARA reserves the right to reschedule, modify, or cancel an
appointment when reasonably necessary, including for clinical,
operational, technological, staffing, or other legitimate reasons. Additional appointment, cancellation, late-arrival, and no-show
policies may apply and may be provided separately to patients. Certain SOZARA services may be offered through membership programs,
packages, programs, subscriptions, or other recurring arrangements. The specific terms governing a membership or program—including fees,
billing frequency, minimum commitment, cancellation, included services,
exclusions, credits, expiration provisions, and other conditions—may be
contained in a separate Membership Agreement, Financial Policy, Program
Agreement, or other patient agreement. If there is a conflict between these Terms and a separate written
agreement governing a specific healthcare service or membership, the
specific agreement will control to the extent of the conflict. Membership fees do not guarantee any particular medical outcome,
diagnosis, treatment, prescription, procedure, or other healthcare
result. Unless expressly stated otherwise, SOZARA operates on a cash-pay
basis and does not submit claims to health insurance for routine
services. However, some services are eligible for HSA and FSA. It is the
patient’s responsibility to check with his/her HSA/FSA provider to
confirm. Fees for memberships, consultations, programs, procedures, products,
laboratory services, medications, or other services may vary and may be
changed from time to time. Prices displayed on the Website are subject to change and may not
include third-party charges, laboratory fees, medication costs, pharmacy
charges, or other expenses unless expressly stated. Patients are responsible for charges applicable to services they
authorize or receive. Additional financial terms may be governed by SOZARA's Financial
Policy, Membership Agreement, or other applicable agreement. The Website does not guarantee that any prescription medication,
compounded medication, controlled substance, hormone therapy, peptide,
supplement, or other product will be prescribed or recommended. Prescribing decisions are made by an appropriately licensed
healthcare professional based upon the patient's clinical circumstances,
applicable law, professional standards, and the provider's independent
clinical judgment. A request for a medication or treatment does not create an obligation
for SOZARA or its healthcare professionals to prescribe or provide that
medication or treatment. The Website may contain information regarding nutrition, exercise,
wellness, longevity, functional medicine, aesthetics, weight management,
hormones, peptides, supplements, medications, and other health-related
subjects. Such information is provided for general educational purposes and may
not apply to your individual circumstances. Medical science and healthcare recommendations may change over time.
